Primary Care Network (PCN)

Hyde Primary Care Network (PCN), known as Healthy Hyde, comprises a group of eight GP practices that work collaboratively to improve patient services and staff wellbeing. The eight Practices are:

  • Awburn Medical Practice
  • Clarendon Medical Centre
  • Donneybrook Medical Centre
  • Dukinfield Medical Practice
  • Hattersley Group Practice
  • Haughton Thornley Medical Centres
  • The Brooke Surgery
  • The Smithy Surgery

We provide services to a population of around 72,000 patients. Some of this is done in partnership with third-party providers, and some of our PCN staff are employed by the PCN, enabling staff to work freely for all eight practices.

Hyde PCN has two Clinical Directors: Dr Jane Harvey of Dukinfield Medical Practice and Dr Fazal Alam of Clarendon Medical Practice. Dr Harvey and Dr Alam are responsible for the PCN’s strategic leadership. Each member practice has a GP on the PCN board.

We also have a PCN Manager, Sally Culmer and a team of wellbeing practitioners, nurses, paramedics, mental health nurses, first contact physios, and care coordinators who work with your community in multiple ways. As a PCN, we are passionate about enhancing and improving the health and well-being of our community. Through collaborative efforts with our member practices and dedicated healthcare professionals, we strive to implement innovative strategies and deliver high-quality care that positively impacts the lives of our patients.

What is a Primary Care Network (PCN)?

PCNs are a group of GP practices that work together with other healthcare staff to provide integrated services to the local population. PCNs can be seen as an extension of your GP practice, and the services that are offered through the PCN are designed to complement the GP practice and secondary care offerings.

What does our PCN offer?

Hyde PCN hub is located at Healthy Hyde Body & Mind, Clarendon Square Shopping Mall, Hyde, SK14 2QT

At the PCN there is a variety of clinical staff providing services to our patients:

  • Adult Mental Health Nurse
  • Musculoskeletal Practitioners (MSK)
  • Paramedics
  • Social Prescriber
  • The Complex Care Team
  • Wellbeing Coordinator
  • Wellbeing practitioners

Extended Hours 

Our PCN runs our extended hours programme where we offer GP, Paramedic, Advance Clinical Practitioner and Healthcare Assistant appointments. The clinicians working in extended access are mostly from our local practices and will be able to access your GP records the same as seeing a clinician at your own practice. Extended hours run from 18:30 – 20:00 every weeknight and 09:00 – 17:00 every Saturday *Excluding bank holidays*

Additional Services

The PCN also runs ad-hoc clinics in and out of normal working hours, such as Urgent Access (same day) appointments and covid and flu vaccines.

How to access these services

For some of the services offered (Mental health, MSK, GP, HCA and Nursing appointments), you can contact the practice, and we can make an appointment for you.  Other services, such as children and adult well-being and social prescribing, require a referral. To discuss a referral, please complete our patient triage forms or contact reception.
