How to Register with Our GP Practice

Registering with our GP practice is simple and ensures you can access healthcare when you need it. Follow these steps to complete your registration:

  1. Check If You Live in Our Catchment Area

Our practice accepts patients who live within a specific area. You can check if your address is covered by entering your postcode in the search box.

We also accept out-of-area registrations for patients who live in Tameside. If you live outside our usual area, please note that we cannot provide home visits for out-of-area patients.

  1. Collect a Registration Form or Register online

To register, you will need to complete our registration form. You can:

The form will ask for details such as your name, date of birth, address, and NHS number (if available).

  1. Provide Proof of Identity and Address (If Required)

We may ask for proof of identity (e.g., passport, driving licence) and proof of address (e.g., utility bill, bank statement).

  1. Attend a New Patient Health Check (If Needed)

Some new patients may be invited for a short health check to collect basic health information and discuss any ongoing conditions or medications.

  1. Confirmation of Registration

Once your registration is processed, we will confirm your registration. After that, you can make appointments and access our healthcare services.

Your named GP

Since April 1st, 2015, all GP practices must assign each patient a named accountable GP. This is mainly for administrative purposes, ensuring that every patient has a designated GP responsible for their care if needed.

  • Your named GP will be Dr Holly McBride
  • If you’d like to know who your named GP is, simply ask at reception during your next visit
  • There’s no need to call the practice—this is not something that affects your day-to-day care

Can I Still See Other Doctors?

 Yes! Having a named GP does not mean you can only see that doctor. You are free to book appointments with any GP at the practice. If your named GP is unavailable or if you have urgent medical needs, you may need to see another doctor instead.

At Dukinfield Medical Practice, all registered patients have been assigned a named GP, but you can still see any doctor you prefer.
