Fit Notes (Sick Notes) – What You Need to Know

 What is a Fit Note?

A Fit Note (also known as a sick note or Statement of Fitness for Work) is a document issued by a healthcare professional, such as a doctor or nurse, to confirm that you are unfit for work due to illness or injury. It can also provide recommendations on adjustments to help you return to work.

Fit Notes are required if you have been off work for more than seven consecutive days (including weekends and bank holidays). If you are off for seven days or less, your employer may accept self-certification, and you will not need a Fit Note.

What Information Does a Fit Note Include?

A Fit Note will state:

  • Whether you are not fit for work or may be fit for work with adjustments
  • The duration of the note
  • Any advice or suggested modifications (such as reduced hours or lighter duties)
  • The name of the healthcare professional issuing it

How to Request a Fit Note

  1. If You Are Already Under Our Care

If you have been seen by a doctor or healthcare professional regarding your condition, you can request a Fit Note by:

  • Visiting the practice reception during opening hours
  1. If You Have Not Been Seen for This Condition

You may need an appointment with a GP or healthcare professional before a Fit Note can be issued. Please contact us to arrange this.

  1. Renewing a Fit Note

If you need an extension for your Fit Note, you can request a renewal by:

  • Using our online request form –  see here
  • Calling the surgery – 01613436382
  • Visiting the practice reception during opening hours

We recommend making your request on the day your current Fit Note expires to ensure continuity and avoid any delays.

Important Notes:

  • Fit Notes can only be issued for medical reasons.
  • Employers are responsible for considering any recommendations made in the Fit Note.
  • We cannot provide Fit Notes retrospectively for periods where you were not seen by a clinician.
