Online Access

You can do the following online once you’re signed up:

  • Order your repeat prescription
  • Order acute medication using secure messaging
  • Request an appointment
  • View your medical records

How to Sign Up for Prescriptions and Appointments:

  1. Read the Online Access leaflet here
  2. Download and fill out the Access Consent form here and give it back to reception.
    (You can get the form at reception too.)

How to Sign Up for Medical Records:

  1. Do the same as above.
  2. Fill out a questionnaire, which you can download here or get at reception.
  3. Return the completed form to reception.

Forms are available at reception if you need them.

How to Sign Up for Proxy Access

 Proxy access lets a trusted person (like a family member or carer) manage your GP appointments, prescriptions, and medical records online.

 Here’s how to set it up:

  1. Check if you’re eligible. You can request proxy access for a child, elderly relative, or someone who needs help managing their health. The patient must agree unless they cannot make decisions for themselves.
  1. Download and fill out the Proxy Access form here or pick up a form from reception
  2. Return the form to reception along with your photo ID (passport or driving licence) and proof of address.
