Patient Triage Step by Step Guide

Requesting Help from a Doctor or Nurse

TriagetileUnder the new system, you no longer ask for an appointment, but instead, you complete a Patient Triage Request describing your problem, how long you have had the symptoms etc and submit it to the Practice.  Your needs will then be given the required priority (triaged) and your details passed on to the appropriate clinician who will then contact you within 48 hours.

Procedure for completing a Patient Triage Request:

Firstly, click on the green tile on the website homepage titled Patient Triage.

On the page that opens, click on the tile titled Request a Consultation.

When the page opens, click on “Request a Consultation”

On the next screen select what you would like help with.
Confirm that you are not contacting the Practice about an emergency.
The next page is where you can describe your medical problems and symptoms.
Please provide us with as much detail as you can in the free text boxes provided. This will help the doctor or nurse deal with the problem accordingly. Only send a photo if it helps to clarify your problem. Please do not send intimate images.

Then click on “Continue

Confirm who the request is for and click “Continue
Complete your details and click on “Continue”. If you can, please verify your details by clicking “Yes, Verify”

Check your answers and then click on “Accept and send information”

Your Patient Triage Request has now been sent to the system to be prioritised (triaged) and you will be contacted within the next 48 working hours.
