Patients can

  • Renew Prescriptions
  • Book Appointments (with Doctors only)
  • View Medical Records

online once they have registered to use the service.

To Register for Prescriptions and Appointments service:

Firstly, please read the Online Access leaflet

then complete the Access Consent form and return it to reception.
Blank forms are available at reception also.

To Register for Access to Medical Records:

In addition to the above, please complete the questionnaire which can be downloaded here and return to reception
